THD – Doppler Assisted Procedure

The THD (Transanal Hemorrhoidal dearterialization) Doppler Assisted procedure, is an effective hemorrhoid treatment that uses a minimally invasive surgical approach to significantly shrink hemorrhoidal prolapsed tissues. It does this by identifying and blocking the blood vessels which feed the hemorrhoids.

King’s College Hospital London UAE is one of the few hospitals in Dubai that offer this treatment procedure.

Causes of Hemorrhoids

Hemorrhoids occur due to the enlargement of veins around the anus. There are several factors that cause this, including:

  • Chronic constipation
  • Diarrhea
  • Pregnancy
  • Heavy lifting
  • Sitting for too long
  • Genetics
  • Obesity

Symptoms of Hemorrhoids

Some of the common symptoms of Hemorrhoids include:

  • Depending of the size of the hemorrhoid, or grade, one can experience severe pain and soreness on the area
  • Itching and irritation around the anus area
  • Bleeding
  • Inflammation
  • A hard lump/bulge on the location of the hemorrhoid (in or outside the anus)
  • Swelling of the anus
  • Painless bleeding

How the THD – Doppler Procedure Works

The THD – Doppler procedure works by reducing the supply of blood to the hemorrhoids by stitching the hemorrhoidal arteries.

During the procedure, the surgeon uses a  doppler probe device to help him locate the position of hemorrhoidal arteries through the back side (anus). The said blood vessels are usually located above the hemorrhoids on the rectal wall. The surgeon then ligates the hemorrhoidal arteries with sutures, which then stops the blood flow to the hemorrhoid. This causes gradual shrinkage of the offending hemorrhoid, hence drastically reducing the current present symptoms of hemorrhoids.

In cases where the is a prolapse of the hemorrhoids, which means the tissues have protruded to the outside of the anus, then the surgeon may perform a haemorrhoidopexy – a procedure whereby extra stiches are placed to lift and pull these offending tissues back into the back passage (anus).

How long does the THD – Doppler procedure take?

The procedure, which is done under general anesthesia, is a day case procedure, which means you may be discharged on the same day, but in some instances, you might stay overnight especially if the procedure has been performed in the evening. Overall it takes approximately 40 minutes, but actual length of the procedure will depend on each individual case.

Is the procedure painful?

Most people who have undergone the THD – Doppler procedure usually report some degree of fairly low discomfort afterwards. The downtime of the procedure is also very minimal.

Benefits of the THD – Doppler procedure

  • Post-operative pain is lower since there’s no excision of tissue
  • There are no open wounds left behind
  • It is minimally invasive
  • It has minimal downtime
  • It has lower complications
  • Relieves hemorrhoid symptoms in the short and long term

THD – Doppler procedure complications

The procedure can have minimal complications, although a majority of the patients get on well post the procedure. However, some of the complications one might experience include:

  • Mild anal or lower abdomen discomfort which may last for up to 7 days
  • Urgency to move bowels, which resolves in less than 7 days
  • Possibility of passing blood-stained bowels
  • Difficulty in passing urine (although this is rare, and may happen to men with an already pre-existing prostate condition)
  • Possible recurrence of symptoms or prolapse of the hemorrhoidal tissues
